MFC 1.72-1.74: Fix a bogon the range checking for reading the LDT list
and handle some SMP races in i386_set_ldt() by copying in the new LDT
entries in one go rather than doing it piecemeal.

Tested with:	wine running notepad over a windows partition mounted via
		NFS over UDP over an 802.11(b) connection from my laptop.
		It was a bit slow.
