- Remove explicit enabling of the BSP's APIC in the APIC_IO case and the
  slave pin on the master PIC in the !APIC_IO case.  The PIC drivers now
  manage these details internally.
- Remove an spl0() that hasn't done anything since SMPng was first
  committed.
- Update some comments that have rotted since SMPng.
