Remove cache flush, 256Mb 1:1 BAT mapping, and BAT clearing. This doesn't
work on a G5 (no BAT registers) or on PearPC (dBAT3 used for mapping
the framebuffer and BATs not re-inited on OpenFirmware calls).

 It also hid a number of bugs.
