Cumulative MFC of patches applied to -current to get preemption
support to a non-instantly-crashing state.
Also includes some cosmetic changes to keep the diffs to a minimum and
an experimental option (followon) in sched_4bsd. (turned off but
included to keep diffs to a mimimum)
