MFC changes from current:

- Prevent ndis_get_info()/ndis_set_info() from possibly sleeping on
  the same wait channel from two different threads
- Protect calls to MiniportQueryInformation()/MiniportSetInformation()
  with a spinlock
- Export ntoskrnl_init_spinlock() from subr_ntoskrnl
