Various locking fixes and mark MPSAFE:
- Add locked variants of start(), init(), ifmedia_upd(), and poll() and stop
  recursing on the driver lock.
- Add locking to ifmedia_upd() and ifmedia_sts().
- Use callout_*() instead of timeout/untimeout.
- Fix locking in ioctl().
