Commit 8/14 of sched_lock decomposition.
 - Use a global umtx spinlock to protect the sleep queues now that there
   is no global scheduler lock.
 - Use thread_lock() to protect thread state.

Tested by:      kris, current@
Tested on:      i386, amd64, ULE, 4BSD, libthr, libkse, PREEMPTION, etc.
