MFC ISO9660 improvements from r176403 and r179321:
  * More portable substitute for platforms that lack timegm()
  * detect buffer allocation overflow
  * don't be confused by zero-length files (such as symlinks or device nodes)
    with garbage offsets; this seems to fix most instances of "Ignoring
    out-of-order file"
