Removes `usb_bus_mem_callback_t' and simplifies the logic passing the
callback pointer.  As a refection by this change the following prototype
of functions are changed:

  o usb_bus_mem_flush_all
  o usb_bus_mem_free_all
  o usb_bus_mem_alloc_all
