MFC: r213894, r213896, r214913

Converted the remainder of the NIC drivers to use the mii_attach()
introduced in r213878 (MFC'ed to stable/8 in r214685) instead of
mii_phy_probe(). Unlike r213893 (MFC'ed to stable/8 in r214909) these
are only straight forward conversions though.
