Make g_raid_md_intel_start_disk() a single entry point for any new disk.
Make it replace FAILED or OFFLINE disks with new ones. Together with
other changes this allows to use STALE and potentially SPARE disks to
refill array after ACTIVE disk failure or unplug/disappearance.
Remake `graid insert` to also use this function.
