 - Consolidate duplicate code into support functions.
 - Split the bqlock into bqclean and bqdirty locks.
 - Only acquire the wakeup synchronization locks when we cross a
   threshold requiring them.
 - Restructure the way flushbufqueues() targets work so they are more
   smp friendly and sane.
