r254466 bumps up User and Kernel VA considerably, which upsets our
current dmap shim with 4K pages. We differ this until a proper
implmentation is done for xen.

These changes are with a view to sync-ing the branch with head
and thus breaks functionality considerably. The console for example
is now visible only via the hypervisor debug console, because
the interrupt bootstrap has been affected by the underlying churn
in the HVM port. This and other breakage will presently be addressed.
