Work around FIFO bug in SMC UARTS.

Fix endless loop in siopoll() for an event on a tty with no tty struct.
Don't generate unwanted interrupts in the serial console driver.  These
bugs probably don't matter unless the tty struct is dynamically allocated.

Support polled mode.  To use it, leave out the irq and the vector in
the config file.  It only causes extra overhead for open polled ports.
The maximum usable speed is approximately 1000 bps for a 16450 and
15000 bps for a 16550.

Other cosmetic changes.

Re-work the way pre-install was being depended.  I couldn't make the
install cookie work any other way (perhaps I'm just being stupid).
In any case, INSTALL_COOKIE now works as advertised, and prevents duplicate
installations.  pre-install users will have to keep their own cookies if
they wish to avoid duplicate installations, or tell me how to make the
rules run properly.

The code I wrote to write mbufs out using PIO had a bug in the handling
of mb_offset given the right sequence of 1 and 0 byte mbufs. This bug
was discovered by John Hood who also provided this fix -  which is a
rewrite of the routine (and is easier to understand than the code I wrote).

I got a lot of
"getty repeating too quickly on port %s, sleeping"
from init.bsdi, it means that getty start and exit in five seconds.
This is common situation for poor quality Russian phone lines:
modem got CONNECT message and after retries got NO CARRIER.
So I introduce spacing count, it means that this warning and
sleep occurse only after GETTY_NSPACE times of sequental attempts.

I found (and fix) ugly bugs in init.bsdi (this bugs not present
in old init)

1) Init don't setup TERM environment variable for default terminal
type from /etc/ttys before calling getty/window.

2) When "kill -1 1" issued, init don't restart getty when
/etc/ttys parameters was changed (it only kill "off" end empty entries).

3) Small memory leak if "window" /etc/ttys parameter specified and
"kill -1 1" issued.

For those who want a little more control over their build process...

Added two new targets, `most' and `mostinstall', which compile and
install (respectively) just the binaries from just the ``core'' parts
of the system.  This makes it easier to do something like the following
(which I do):

	# cd /usr/src
	# make obj depend
	(scrutinize make output)
	# make most
	# make mostinstall

...which will not confuse things by installing new libraries, /usr/share,
games, gcc (two extra times), and so on.  Obviously, if you do this, then
you have to be careful to watch for changes in include files and make macros,
and do the appropriate thing in those cases.  (It usually still doesn't
involve building GCC three times or libc six.)

unix disklabel in any existing DOS partition while retaining the
existing mbr bootcode or we can install to the whole disk which

   One of the alpha testers (ETO, Toshihisa <eto@osl.fujitsu.co.jp>)
of my APM driver sent me a very small patch to if_ze.c for using IBM
PCMCIA Ethernet card II.  There are only a few difference between
Ethernet card I and II.  So we can use them both with this patch.  It
also includes a patch for PCIC of ThinkPad 230Cs (As long as I
remember, this model is available in Japan only. But it is very
popular subnote in Japan).
