Extend ap_boot_mtx scope to also cover mca_init().

Otherwise, under bootverbose, the lapic_enable_cmc() banner 'lapicX:
CMCI unmasked' is printed by several CPUs in parallel, causing garbled
output for the LAPIC dumps.
