Integrate the new "nanokernel" PLL from Dave Mills.

This code is backwards compatible with the older "microkernel" PLL, but
allows ntpd v4 to use nanosecond resolution.  Many other improvements.

PPS_SYNC and hardpps() are NOT supported yet.
