* Merge gdb/doc/Makefile into binutils/doc/Makefile
* Update build for gdbserver and gdbreplay to work under binutils
* Fix gdbserver to use PT_GETREGS etc to access registers, removing the
  dependancy on the u-area.
* Make gdbserver work on the alpha.
