MFC: dmasetup now uses u_int16_t's for request queue indices. Regularize
and deal with f/w availability or lack thereof. Dig chip revision out
of PCI header and store in softc. Generate WWN from config'd SCSI_ISP_WWN
or get it from a quad in the environment. If all else fails, try and generate
a default WWN that is vaguely sensible.
