- Fix the sw_alloc_interlock to actually lock itself when the lock is
  acquired.
- Assert Giant is held in the strategy, getpages, and putpages methods and
  the getchainbuf, flushchainbuf, and waitchainbuf functions.
- Always call flushchainbuf() w/o the VM lock.
