mike@ has been inactive for about a year, so make some more updates to
the C99 webpage:
- nectar took care of getpwnam_r(), getpwuid_r()
- bms implemented posix_madvise().
- I implemented fenv.h.
- tjr finished wcscoll() and wcsxfrm() and wide-character conversions
  for scanf().
