Update to 2.2.2.

Changes:
  - Default KEY_SIZE to 2048 bits
  - Default the signing hash to SHA256 rather than SHA1 and MD5
  - vars cleanups
  - pkitool cleanups
  - pkitool -days fix for https://community.openvpn.net/openvpn/ticket/198
