www/mod_auth_mysql_another: cleanup / apache24 fixes

- Bump PortREVISION
- Set LICENSE
- Allow apache 22+
- DAPACHE2 isn't needed anymore
- consolidate post-patch and patches into one
- unmute install command
- remove unused files/Makefiel
- handle remote_ip vs client_ip
