misc/terraform: PKGNAMEPREFIX and cleanup

Add PKGNAMEPREFIX so I can use the package name with sysutils/terraform
While here, pet portlint by removing duplicate PORTREVISION and sorting USES
Bump PORTREVISION for package name change
