Update to 0.47.

Move p5-Test-mysqld to TEST_DEPENDS. It can be used but isn't required. Add
p5-Test-postgresql to TEST_DEPENDS as well. They could probably be reasonably
added as OPTIONs. Also add a note that to run all the tests you need to add
USE_MYSQL=server.
