Update to version 11.31

Release highlights:
 - QS_SetReqHeader supports header removal (unset) by prefixing
   the header name with "!".
 - QS_SetEnvIfResBody supports variable removal (unset) by
   prefixing the variably with "!".
 - New cookie data format (user tracking, vip, milestones).
