portsdb(3):
- Fix the EXTRA_CATEGORIES support that was broken in recent updates.

pkgtools.conf(5):
- Add a note that a user must run portsdb -Ufu to reflect values of
  EXTRA_CATEGORIES and IGNORE_CATEGORIES.

For the victims of the above bug, please run "portsdb -qu" and then
"portupgrade -q portupgrade".
