Update the maintainer's address (I remember him asking his address in
the handbook to be updated.)

Style fix:
- Insert a blank (`3' only) line before the $Id$ line.
- Move EXTRACT_SUFX to proper place.
- Regenerate the patch file and remove the definition of PATCH_STRIP.
- Correctly define MANLANG and MAN1.
